Basic Service:

We’ll have an initial interview consisting of the following:

. Brief Health History

.Identification of current health issue/ health goals

.Identification of client/family questions

Statement of fee & method of payment

.Identification of time & location of upcoming doctor’s/ procedure appointment

I will meet you at the medical facility & accompany you to your appointment.

.At the end of your appointment I will accompany you to the meeting place for your transportation and then I will leave.

I will follow up with you to clarify any questions and determine the need for my future services.
